Job Summary: The Coder is responsible for coding medical records, including all diagnoses and operative and diagnostic procedures, using current ICD, CPT, and HCPCS codes, and entering coded information into an automated grouper system.

Responsibilities:

1. Assign and record accurate codes for diagnoses, procedures, and operations based on medical documentation.
2. Ensure all factors necessary for accurate DRG assignment are present and diagnoses are properly ranked.
3. Make contacts to clarify or acquire necessary information.
4. Enter diagnostic codes and descriptions into an automated system.
5. Respond to inquiries from hospital staff regarding medical documentation and coding.
6. Abstract information from medical records for reports and statistics.
7. Enter data such as diagnosis, treatment, admission/discharge dates, and length of stay into hospital databases.

Qualifications/Requirements:

• Two years of outpatient medical records coding experience in a hospital setting.
• High school diploma or equivalent; an Associates or Bachelors degree in health information management may substitute for one year of experience.
• Current certification as CCS, CCS-P, or CPC.
• Thorough knowledge of HCPCS, CPT, ICD codes, and medical terminology.
• Effective communication skills and proficiency with computer applications.
• Ability to read, write, speak, and understand English sufficiently.
